New Family Consultation

We offer free 30 minute consultations to new families. This provides you with the opportunity to speak with an Intake Coordinator, ask any questions you may have and learn about what makes CI unique as a therapy provider. Intake paperwork

After the consult, the next step is to complete our intake paperwork. With your permission on the intake paperwork, we will call on your behalf to request a referral from your physician’s office if needed for the service you’re pursuing. We will also investigate any insurance coverage options that you have access to. We pride ourselves on always taking these extra steps for our families.
